This form has to be completed in order for a formal complaint to be registered against a member of the SAIA in terms of its Code of Conduct.

This form must be completed to formally register a complaint against a member of the South African Insurance Association (SAIA) under the terms of its Code of Conduct. The SAIA is committed to upholding the highest standards of ethical conduct and ensuring accountability among its members.
